As far as I'm concerned there hasn't been any of what you mention, just the usual kind of thing one would expect from any large organisation. You are obviously like Nelson using his blind eye to use the telescope and saying I see no ships! As far as you are concerned they can do no wrong so no surprise in your answer! I can see that you have very little expectations in your life if this is the standard you would expect from any large organisation. Just to remind you they are not large organisations. One is a local town council, and the other similar but slightly larger. Some Councillors serve on both, may go some way to explain why they suffer very similar problems, and of course with humans involved you will always get mistakes but to not admit to them and then try to blame the taxpayers who point the mistakes out is simply not acceptable whatever the size of the organisation. You must have a lonely hobby trying to defend the indefensible?
