I think are some who choose not to accept help that comes with certain caveats, and I am sure there are some who made bad choices which led them to their situation. But it flies in the face of human nature to make life hard for yourself.

To some it's a way of life in afraid, there's also a lot out there with mental health issues, many, as you say, don't want conditions heaped on them by people who really should know better. Southampton and Portsmouth are quite good with their outreach programs, many staffed by volunteers, there's even a vets service to help their pets to cope.
